Respite and Habilitation FAQ’s
Who is eligible for Home & Community Based Services?
Any person receiving services through Division of Developmental Disabilities (DDD), and Arizona Long Term Care (ALTCS), is eligible for HCBS services. Contact your DDD Support Coordinator to see if you qualify.
What HCBS services does Milestone Pediatrics provide?
Respite:
Respite care is primarily used to give caregivers a break from the emotional demands of providing continuous care to someone who has a disability. Respite care services include self care skills, social skills, assistance in communication, bathing, and toileting. Respite care may be used hourly and daily.
Habilitation:
Habilitators are responsible for providing care to clients based on their current set of goals and level of functioning. It is used to increase independence in individuals and offer support services including improving communication, learning adaptive skills, independent living, and any other goals set by the support team. Habilitation may be used hourly.
Can my Provider watch my other children in the home?
No, they cannot. Milestone Pediatrics is only authorized to provide services to the child receiving services through DDD.
Can I select my own provider?
Yes. All families are encouraged to meet their potential new Provider and interview them to see if it is a personality match, as well as training level and availability match.
Who can I select as my provider?
Milestone Pediatrics will happily hire your family member, friend, babysitter, child's church counselor, or teacher for your child. They must meet our qualifications, attend our required trainings, attend new hire orientation, be at least 18 years old, and pass a background check.
What are the steps to get my provider started?
• Schedule a 90 minute orientation with our HR Director at our office in Chandler at 480-855-0474
• Submit copies of training certificates and HR paperwork
• Have the family call their DDD Support Coordinator to authorize their Respite hours to Milestone Pediatrics
Which training's does my provider need, and how much do they cost?
Your selected provider needs the following trainings:
CPR/ First Aid
Article 9
Level one Fingerprint card ($67 payable by the provider)
The required trainings are free of charge and can be taken at our office. However, there is a fee to obtain copies of the training certificates, and can be purchased at any time at our office, if needed.
Your Provider may also obtain their trainings at the following locations: (Trainings taken outside of Milestone Pediatrics are not reimbursed.)
CPR/First Aid ($50)
Article 9 ($30)
Fingerprint Card ($67 + $7.95 scan fee)
